IMPORTANT Please read before downloading REFERRAL FORMS
Instructions for Using Discovery Program Referral Forms and Checklists

Please follow these guidelines when using Discovery Program Referral Forms and Checklists:

  • Open the Discovery Program Referral Forms link in this email. This is a Share Point link.
  • Click on the form you need.
  • Download the form into your One Drive BEFORE entering any data.
    (It’s a good idea to download all of these forms and save them in your One Drive for accessibility.)
  • Close Share Point.
  • Print out the form, if you wish to hand write your information as you observe student.
  • Enable Editing on the form to enter your information electronically.
  • Save a copy of your completed form in your files.
  • Share your completed form as an attachment via email to Toni Pudlo.

WHEN COMPLETING FORMS:

  • Include all information that is pertinent to a student’s progress. Be as complete and factual as possible with objective observations and data. Provide student work copies if possible.
  • Be honest about a student’s deficits or negative habits.
  • Include information that you would expect to see in a formal educational record. Do not include vague observations, impressions or opinions.
